Purpose

How does your facility promote inter-professional collaboration during times of patient transitions?
Hand-over communication is one of the ways our facility promotes inter-professional collaboration during times of patient transitions. During patient transitions, communication is quite crucial for the enhancement of patient outcomes. According to the WHO (2007), a majority of patient hand-over problems arise from the poor quality of hand-over communication. With this in mind, and keeping abreast with the standard of collaboration, our facility seeks to create an environment where caregivers appreciate the value of partnering with each other to enhance patient outcomes.
Aside from hand-over communication, there is also patient and family engagement. Often, caregivers ignore patients and families during patient transitions. The outcomes mainly lead back to acute care for patients and criticism for caregivers. However, as indicated by Kucknall et al. (2016), patient and family engagement could act as a safety buffer and help reduce adverse events that often occur because of ignorance of the above.
