Hypertension disorders constitute some of the challenges that nurses who work in maternal healthcare units will face every day. With ever-rising cases of chronic illnesses like hypertension, maternal and child health will always be at risk, and this means that one of the ways of battling hypertension in pregnancy involves arming the nurses with adequate information and skills on pregnancy-related blood pressure disorders. The management of hypertension in pregnancy has far-reaching implications on nursing practice, including the capacity of the nurses to assist in prevention, screening, diagnosis, and interning during emergency cases, in the context of gestational hypertension, pre-eclampsia and eclampsia.
The hypertensive disorders in pregnancy, which include pre-eclampsia and eclampsia, are among the leading causes of maternal and perinatal morbidity and mortality (American College of Obstetricians and Gynecologists (ACOG) 2020; Macedo et al., 2020). The complications associated with pre-eclampsia covers 2–8% of pregnancies globally (Bouter & Duvekot, 2020), almost 26% of maternal deaths are attributed to hypertension disorders in Latin America and the Caribbean while they lead to about 9 % of maternal deaths in Africa and Asia (ACOG.,2020). Pre-eclampsia is a pregnancy disorder characterized by hypertension and multisystem complications, and adverse outcomes of pre-eclampsia include complicated hypertension, stroke, renal and hepatic injury, hemorrhage, fetal growth restriction, and death (Anderson & Schmella, 2017; Chappell et al., 2019).
. Gestational hypertension is more common than pre-eclampsia, but the later presents profound challenges during pregnancy. Macedo et al. (2020) point out that pre-eclampsia is possibly caused by the trophoblastic invasion failure in the myometrial spiral arteries, and other mechanisms of causation may include immune 13 and epigenetic 14 dynamic changes that can define the onset and progression of this condition, which results in placental dysfunction. However, the genetic component remains unclear.
The pre-eclampsia disease process is complex: its mechanism is postulated to include chronic uteroplacental ischemia, immune maladaptation, very low-density lipoprotein toxicity, genetic imprinting, increasing trophoblast apoptosis or necrosis (, and an increased maternal inflammatory response towards deported trophoblasts, while recent findings suggest the imbalances of angiogenic factors (ACOG, 2020). Pre-eclampsia is confirmed blood pressure ≥140 mmHg systolic or ≥90 mmHg diastolic) after 20 weeks of gestation (Neuman et al.,2018) and is characterized by the presence of proteinuria with or without evidence of acute kidney injury, liver dysfunction, neurological features, hemolysis or thrombocytopenia, in the mother, and or with fetal growth restriction (Macedo et al. 2020). Eclampsia advanced pre-eclampsia, but it is marked by the onset of generalized tonic-clonic seizures in a woman with pre-eclampsia.
