Assessment Phase:
Baby Boy Jones was delivered at 39 weeks gestation by repeat cesarean delivery to a 36 year old mother Amy Jones. Amy Jones is now gravid 2 para 2. Baby Jones' Apgar scores at birth were 7 at one minute of life and 9 at 5 minutes of life. At 1 hour of age, Baby Boy Jones' respiratory rate was 70 breaths per minute with mild intercostal and subcostal retractions and mild cyanosis.
An hour later Baby Boy Jones is now receiving 40% oxygen and has a respiratory rate of 90 breaths per minute. Oxygen saturations are ordered and infant is under a radiant warmer. His oxygen saturation rate was 92% but once he was placed under the oxygen it has been running 98%. Baby Boy Jones has an order to be fed 1 to 2 ounces of formula every 3 hours.
An x-ray reveals a generalized overexpansion of the lungs and has the diagnosis of transient tachypnea of the newborn.
An IV is started on the baby.
Based on the above information, develop a nursing care plan.
Analysis:
Develop one (1) actual nursing diagnostic statement based on presented data that identify health problems. Consider the information you have related to the newborn's transition to extrauterine life and the type of delivery of the mother.
Planning:
Develop one measureable patient outcome that will prevent, reduce, or resolve the identified patient health problems (nursing diagnostic label).
Implementation:
For the nursing diagnostic statement, write four (4) nursing interventions. A maximum of two of the nursing interventions can be assessment based.
Evaluation:
Evaluate the effectiveness of the nursing interventions and write an evaluative statement indicating that Baby Boy Jones has achieved the expected outcome.
Baby Boy Jones has resolution of his symptoms 48 hour later.
