Scenario: Michael, a 35-year-old was brought via ambulance to the emergency department after collapsing on the street. He was diagnosed with appendicitis and the surgical team was alerted to the potential surgery. The physician prepared to obtain informed consent and began discussing the simple effective surgery and the treatment with the patient. The RN was present at the bedside. The patient stated he did not want surgery. Based upon his beliefs as a Christian Scientist, it is against his practices. He requested a Christian Science practitioner. The patient rated his pain as 9 on 0-10 verbal pain scale so the RN prepared the narcotic analgesic to relieve the patient's pain. The patient declined the medication. The RN believes the patient should accept the pain medication and have the surgery thinking βIf it were me I would proceed with the surgery and treatments proposed by the surgeon.β The surgeon can be heard speaking to a colleague about how to go about changing the patient's decision.

Patient`s best interests should always be given the first priority in any medical environment. However, many patients sometime don`t understand this and only want their decisions to be executed (Waxler, 2005). In some cases, the doctors, surgeons or registered nurses (RN) views are contradictory on what the best interests of the patient are. It is the duty of the medical practitioner to make sure that the patient`s best interests are looked at any particular event.
When determining the best interest of the patient, the RN is supposed to look at a variety of factors that will aid in providence of better health care. First, the RN should establish a communication link with the patient (Galanti, 2007). This will bridge the gap between the patient`s beliefs and practices and the medical culture thereby making him value the system of medical administration. The patient should be made to understand that being sick is not an indication of the ills in the physical world but an inevitable event in human life and more than just prayers is needed for him to get better.
