One intervention that the primary care advanced practice nurse will include in the survivorship plan of care is future check-ups and follow-up. Future check-ups and cancer tests would be important to assist in checking other health problems that may occur weeks, months, or years after the treatment ends. Ideally, check-up and tests are essential to determine if the cancer has come back or other types of cancer have developed (Weaver, Jessup & Mayer, 2013). In this way, follow-up care with check-ups and tests is essential in ensuring that any health problems that may occur after treatment are caught in time.
Another intervention the nurse will include in the survivorship plan of care is potential long-term late effects of the treatment that the patient has received. This is important because some treatments for cancer such as chemotherapy may have long-terms effects such as nerve problems or difficulty in concentration.
