What labs and screening tools would be appropriate for this patient?
Mr. J should be screened using the generalized anxiety disorder seven items (GAD-7) tool to do screening for GAD. When screening for anxiety disorders, an eight score or more signifies a sensible cut-point for recognizing likely cases of GAD (Byrd-Bredbenner et al., 2020). In addition, there are comorbidities with GAD, such as heart sicknesses, thyroid disorder, and Gastroesophageal reflux disease (GERD). Therefore, Mr. J needs to have lab tests such as chest X-rays to rule out cardiac disease, a blood test for detecting thyroid hormone levels, an endoscopy X-ray of the digestive system for GERD, and a trial that has stress levels.
What is the likely diagnosis? What assessment data support your preliminary diagnosis?
Mr. J has GAD with heart disease as a comorbidity. The assessment that supports my diagnosis is that Mr. J experiences difficulty sleeping and staying asleep because he experiences disturbing dreams concerning the poverty and deplorable conditions of people he saw during their mission in South America. The sleep disturbances made him anxious and distracted because he could not concentrate on his work (Hallion et al., 2018). in addition, Mr. J has constant irritation because he feels overwhelmed by anxiety.
How will you, as the PMHNP, separate comorbidity from complications of the current diagnosis?
I would check Mr. J's blood pressure and do a blood test to evaluate his cholesterol level and Electrocardiogram (ECG) to determine if he has heart disease. This will enable me to know if heart disease is also a trigger for anxiety.
