Findings from Collecting the Preliminary Client’s Information
An assessment of the client based on the Initial Assessment and Triage Questionnaire revealed no alarming health conditions, with the only concern being to maintain the increasing body weight to match the doctor’s recommendations for well-being. The client’s goals included improving their appearance, physical fitness, embracing healthy eating habits, and losing the excess weight. The client desires to reduce his high intake of calories by improving his poor eating habits characterized by his high consumption of a variety of fast foods for his meals. The client wants to adopt a healthy eating habit and complement the same with routine physical activities. It also came to my attention that the client comes from a family with a history of obesity, and the fact that he is physically inactive is a risk factor for following the same unhealthy trend. A summation of the assessment identifies the client’s main objective or goals as becoming healthier and gaining physical fitness.
Client’s Level and Reason for Choosing the Level
An analysis of the client’s assessment indicates that he has little knowledge and understanding of the importance of embracing a nutritional wellness regimen, which qualifies him for level one nutrition and coaching model CITATION ISS19 \l 1033 (ISSA, 2019). The client’s physical inactivity is also reflective of the client’s need for fundamentals of nutrition and coaching, as defined in level one of the same.
