Assignment Overview
James and Lucille
James and Lucille met at a holiday party, and it was love at first sight. After four years of marriage, during which the two co-parented James' two sons from his first marriage, the couple decides that they would like to expand their family by having a biological child together. A year goes by, and they have not been able to conceive; the two become concerned.
Lucille schedules an appointment with her primary care physician to discuss her situation. She is subsequently referred to an infertility specialist (specifically a reproductive endocrinologist or "RE"). The couple meets with the specialist, who ultimately determines that the following factors are preventing the couple from achieving pregnancy (see common cause of infertility and male factor infertility):
James, who underwent a vasectomy reversal in the previous year, is now making antibodies to his own sperm. But, according to the RE, should the couple decide to undergo infertility treatment, this is something that could be overcome with the use of an enzyme that halts the action of the antibodies.
Lucille has extensive endometriosis, which her RE indicates will need to be addressed before proceeding with infertility treatment. Six months go by, during which time Lucille's endometriosis is treated, and she is now ready to attempt conception. On the advice of the RE, the couple decides to undergo in artificial insemination (also referred to as intrauterine insemination (IUI)). After three unsuccessful attempts with IUI, the couple, in consultation with their RE, makes a decision to undergo in vitro fertilization (IVF). Five years later, during their fourth IVF cycle, the couple learns that Lucille is now pregnant, and 39 weeks later, the couple's third son makes his entrance into the world.

Infertility causes emotional stress resulting to fear, loneliness and anxiety among others. Infertility also lowers ones self-esteem making couples rethink about continuing with their relationship. According to Reed (2001), infertility causes depression among women within the reproductive age. Even when undergoing infertility treatment both women and men witness high anxiety levels since most couples feel frustrated for not being able to conceive the normal way for them, for the infertility is dehumanizing (Reed, 2001).
Furthermore, hormonal changes due to depression and anxiety, especially in women enhances feelings of being powerless and not living up to the society's expectations .Most men feel embarrassed having to masturbate to give out semen sample. This process itself makes him feel stressed (Christiano, 2016). Infertility treatment itself can be psychologically and physically demanding for couples especially those who are married. The idea that even after treatment the couple's chances of getting pregnant can be minimal is an experience that leads to emotional trauma. Being in a situation where one’s sexual life will be determined by a calendar and temperature charts makes one feel powerless and vulnerable (Christiano, 2016). Issues of hormonal injection on daily basis and producing blood samples and even ultrasound procedures are some of the main contributors to stress. Having to face the fact that they are infertile can affect their social lives and even careers. Many couples would prefer avoiding their friends, families and fearing to attend social events .Using certain modern treatments options like donor sperm insemination can be an uncomfortable experience (Reed, 2001).
Couples mostly women might be withdrawn by the fact that she cannot conceive .This might result in lack of interest in participating in health initiatives while others may feel fearful of having to use donor sperms or opt for adoption. The couples will be much involved in the wanting know what the causes of infertility and how it can be cured making them over stretch their resources. However, this process can help the couples improve their knowledge and actively participates in several awareness education programs (Reed, 2001)
2 How might Lucille's general practitioner support the couple as they undergo infertility treatment?
Lucile's health practitioner can be a good source of information that can guide them about treatment options. The specialist can assist the couples with emotional concerns they might be experiencing during that time because of the treatment they are undergoing (Lykeridou, Gourounti, Deltsidou, Loutradis, and Vaslamatzis, 2009).
It's important for the health practitioner to inquire during counseling if they would be willing to continue counseling to minimize anxiety and other doubts they might be having. The specialist needs to be updated about the treatment progress of the couples to assist where necessary.
