Mrs. A is a 71 year old widow with CHF and osteoarthritis who has recently been exhibiting quite unusual behavior. Her daughter is concerned about her mother's ability to remain independent and wishes to pursue nursing home admission arrangements. She fears the development of a dementing illness. Over the last two to three months Mrs. A has become confused, easily fatigued and very irritable. She has developed disturbing obsessive/compulsive behavior constantly complaining that her lace curtains were dirty and required frequent washing. Detailed questioning revealed that she thought they were yellow-green and possibly moldy. Her prescribed medications are:
Furosemide 40 mg daily in the morning
Digoxin 250 micrograms daily
Paracetamol 500 mg, 1-2 tablets 4-hourly PRN
Piroxicam 20 mg at night
Mylanta suspension, 20 ml PRN
Coloxyl 120 mg, 1-2 tablets at night

Mrs. A, a woman aged 71 years old, has been exhibiting annoying behavior and confusion. She is independent, and her daughter is worried and questions her mother’s ability to take care of herself. She has been prescribed more than five medications, which puts her at higher risks of adverse medical outcomes. Additionally, this causes drug to drug interactions or disease to drug interactions. It may also result in cognitive impairment issues which come along with dementia. The use of these multiple medications might be possibly the reason why she is having visual problems. Managing her medications may be an intense challenge for her. If she takes her medication incorrectly, she may develop electrolyte imbalances which lead to serious health complications. If she takes more than 20mg of Piroxicam with her confusion, it may propel congestive heart failure.
Aging impacts the disposition of drugs; it leads to pharmacokinetic changes, which involve processes such as absorption, distribution to other body chambers, metabolism, and excretion. An increase in gastric PH is common in older adults, and it results in changes in drug absorption. On the other hand, Serum-albumin decreases with age, which affects protein binding; since it may build up toxic drug effects, especially in drugs with a high risk of toxicity (Ruscin & Linnebur, 2019). Hepatic metabolism decreases with age, and this consequently results in high drug concentrations, which also causes toxic effects. Aging is linked to decreased renal elimination. Older adults are less physically active than younger adults, and they have less body mass; hence they produce less serum creatinine. These changes in serum creatinine levels decrease the renal elimination of many drugs.
