I am writing this application form to inform you of my condition. After going to the hospital for many months late last year and other additional months this year my doctor discovered that my health was ailing. I have been suffering from depression and insomnia for a while now. Although I have been visiting the hospital routinely without necessarily having to take a leave, the doctor has advised me to take a leave from work from date 27 August- 30th of September. The doctor has said that during my leave days he will take me through various therapy sessions to assess my improvement. The sessions may take long hours and I need enough time to rest.
