Safety concerns begin the moment a patient enters a health care facility to the point at which the patient is discharged. Yet there are many points where quality and safety can break down. Consider what Dr. Arthur mentioned in this week’s video presentation about health care professionals being the vectors and how many patients acquire additional illnesses during their time at a health facility. As discussed in this week's resources and media presentations, policies and reimbursement strategies are attempting to improve patient safety and the quality of care. One of the key problems that threaten the safety of the patients in the hospitals, at home and in the community settings is medical errors (Caldwell, 2010). Physicians and nurses are trained and in most of the cases are expected to provide the right health care services. However some of the time they fail and this result in harm to the patients. Under this category is the aspect of diagnostic errors. In most of the cases it is common to find that when a patient falls sick, the community and the family members that are assisting will have a preformed diagnosis of the illness (Ballard, 2003).
