Part I: On Gathering Patient Information
A psychiatric mental health nurse practitioner requires competence, commitment, and compassion. Competence is essential in gathering extensive clinical information from the patients to ensure that all the problems will be addressed with the correct or best interventions. Commitment is critical in ensuring that the patients receive a high quality of care throughout their medical care. Compassion is necessary to allow the health practitioners to relate with the patients' suffering and consider their views and opinions.
Among these three requirements, competence is the factor that can be learned from inside the classroom and enhanced by practicing the skills via actual patient encounters. Competence is critical in gathering patient information, which is the most significant aspect of patient-centered care. Gathering detailed and extensive patient information is a prerequisite in making an informed decision. This is secondary to three primary reasons. First, detailed patient information allows the health providers to understand the pathophysiology of the patients' condition, allowing them to decide on the proper and holistic medical treatment. A complete medical history involves the investigation of the patients' chief complaint and the other organ systems that might have contributory factors that endanger the patients' health or degrade their overall well-being. This also investigates the previous illnesses that may have residual effects on the patients' current status. Moreover, it provides information on the previous medical or surgical procedures that the patient received and gives information on heredofamilial or genetic diseases. Social and environmental histories are also investigated, expounding on the patients' risk factors such as alcoholic beverage drinking, cigarette or v...
