The planned change theory is a model applicable for patients who have undergone total knee replacement, whereby it is necessary to determine the effectiveness of heparin in reducing risk of deep vein thrombosis (DVT). This model focuses on a three-phase proposition that requires one to learn and replace changes after having identified weakness through unfreezing, change- refreeze model.
