The Health History and Physical Examination

PURPOSE

As you learned in NR302, before any nursing plan of care or intervention can be implemented or evaluated, the nurse conducts an assessment, collecting subjective and objective data from an individual. The data collected are used to determine areas of need or problems to be addressed by the nursing care plan. This assignment will focus on collecting both subjective and objective data, synthesizing the data, and identifying health and wellness priorities for the person. The purpose of the assignment is twofold.

• To recognize the interrelationships of subjective data (physiological, psychosocial, cultural and spiritual values, and developmental) and objective data (physical examination findings) in planning and implementing nursing care

• To reflect on the interactive process that takes place between the nurse and an individual while conducting a health assessment and a physical examination

COURSE OUTCOMES

CO 2: Utilize prior knowledge of theories and principles of nursing and related disciplines to integrate clinical judgment in professional decision-making and implementation of nursing process while obtaining a physical assessment. (POs 4 and 8)

Health History
This is the health assessment conducted on Jared. This interview happened in a park on a Monday afternoon. I carried my picnic basket filled with different kinds of snacks for us to pass time within the interview. Jared is a tall man about 189cm tall, he is a single parent, a father of one daughter, and he is of a Black-American origin. He was born in 1989 in the city of Los Angeles. He is an accountant with a degree from an accountancy school in Los Angeles. He tells me he works with the local government after a long-term unemployment period of more than 7 months. His previous job was a contractual job, working as an accountant in a small business firm that did not renew his contract.
Jared tells me that his parents are health conscious people and they made sure he attended his clinics as a child. Just like any normal child, Jared faced difficult childhood diseases such as chickenpox, normal sore throat and colds. Currently Jared is strong and healthy; he also has no chronic or any other kind of disease. I asked him if he has been involved in any accidents or admitted in any level of hospital and he responds by first thanking the almighty God for his protection because he has never been in any kind of accidents or hospitalized, despite the high rate of crime among the African Americans society.
Jared’s sister has some allergic disorder and she frequently takes her medicines to control the reactions when exposed. He believes that one day his sister will be fine, since there is no other person in his family of five people who has any allergic disorders. He admits to having no allergies to anything whatsoever. Being a man, he regularly eats well and exercises to keep up his body healthy. I could see his fit body since it was muscular and tanned. He ate most of the snacks and fruits I brought for our lunch and after lunch meals. He was not a choosy eater.
Jared never misses his weekly gym classes and not even a single morning run with his young daughter. He often takes the daughter to the library and this has made him love books, especially about healthy eating. He talked about his family in almost every question I asked. He believes that his family is the most important asset he will ever acquire. His father, who is a musician, helped the family shake things off their shoulders through singing and praising the Lord. The father used to sing gospel songs, which is the reason why he never missed church and why he grew up as a Christian.
As a single parent, Jared learnt to keep himself clean and tidy always. He maintains his daughter’s fingernails as short and tidy as his, this is because his mom taught him that cleanliness starts with simple things and those things were his nails and hands. “My mother worked hard at making me whom I am today. I learnt to maintain my hair, nails and hands clean since I was a child”, Jared said. He talked very clearly and had no throat disorders.
