Papers not meeting this requirement by the end of the session will receive a score of 0 (grade of F). Do keep in mind that papers with a lower SI score may be returned for revisions. For example, if one paragraph accounting for only 10% of a paper is cut and pasted, the paper could be returned for revision, despite the low SI score. Please use the report and your SI score as a guide to improve the originality of your work.

CRA500 SLP MOD 2: Change Management Student's Name Institutional Affiliation CRA500 MOD 2: Integrated Healthcare Delivery System The healthcare delivery system of the entire country is fragmented due to "clinical linkage deficiencies". These clinical deficiencies are due to conflicting resources, lack of proper coordination, high costs of living and the ever-increasing government expenditures. The need to eliminate unsafe health care is necessary to improve the quality and reduce the involved costs to make the system financially sustainable. This can only be achieved through an integrated health care delivery system without which the system will be heading to the south (Institute of Medicine of the National Academy of Sciences, 2001). However, few organizations have the knowledge and know the benefits that the system has. The concept of integrated healthcare delivery system has been around for the last seven decades being introduced in the 1930s and gained momentum in the 80s. In the early 90s, it was confused with "managed care" which also continues to date. However, scholars have helped many organizations and leaders to fully understand this concept of integrated healthcare delivery system. They have defined it as "an organized, coordinated, and collaborative network that: (i) links various healthcare providers via common ownership or contract across three domains of integration (economic, noneconomic, and clinical) to provide a coordinated, vertical continuum of services to a particular patient population or community and (ii) is accountable both clinically and fiscally for the clinical outcomes and health status of the population or community served and has systems in place to manage and improve them" (Burns & Muller, 2008). How the organization is transitioning integrated healthcare delivery system The continued staff shortages, increased inflation in the country has called for an effective and efficient use of the available resources through integrated healthcare delivery systems. These systems are believed to be providing higher performance in quality and safety due to effective communication and standardized structures. Despite the demand for implementation of these models, the information on implementation and evaluation of the systems is not easily accessible by most leaders and decision makers. There is little guidance on how to how to implement and evaluate the models. Our organization has managed to overcome the challenges by applying the current knowledge on the integration of health systems to improve service delivery (Intermountain Healthcare, Kaiser Permanente, and Mayo Clinic, 2008). The organization has emphasized on shared values and goals where all the participants are committed to delivering high-quality care to the patients. The management of ...
