Difference between conventional case management and disease management
Conventional case management (CM) basically targets high-risk patients; meaning patients who, due to different combinations of functional, social, and health problems, are likely to need to be hospitalized. These high-risk patients are at high risk of suffering expensive hospitalizations as well as poor health outcomes owing to multifaceted medical and social vulnerabilities (Chen et al., 2009). Case management is, in essence, a collaborative process of evaluation, facilitation, planning, and advocacy for services and options to meet the health needs of an individual through communication as well as available resources that foster cost-effective and quality outcomes (Buppert, 2008). Conversely, disease management (DM) targets patients having a single specific diagnosis and who, due to their major diagnosis, have a comparatively standard set of needs that are related to that diagnosis (Chen et al., 2009). Unlike CM, DM is essentially the concept of trimming down costs of healthcare and improving the quality of life for persons suffering from terminal illnesses by reducing or preventing the effects of the condition by means of integrated care (Buppert, 2008).
