A 16-year-old male presents for a sports participation examination. He has no significant medical history and no family history suggestive of risk for premature cardiac death. The patient is examined while sitting slightly recumbent on the exam table and the advanced practice nurse appreciates a grade II/VI systolic murmur heard loudest at the apex of the heart. Other physical findings are within normal limits, the patient denies any cardiovascular symptoms, and a neuromuscular examination is within normal limits. He is cleared with no activity restriction. Later in the season he collapses on the field and dies.

Cardiovascular diseases are the diseases of the circulatory system and are one of the leading causes of death worldwide. The most threatening heart disease being the coronary heart disease which is the leading cause of mortality in the US. Other heart disorders include stroke, aortic disease and the peripheral arterial disease (Lilly, 2012). Under special cases, abnormal heart murmurs may be accompanied with hypertrophic cardiomyopathy and heart disease resulting in heart attack and eventually death. Innocent heart murmurs as is mostly referred is common in children and is in most cases considered harmless. It is a condition where the heart produces whooshing, rasping or blowing sound during heart beats (Lilly, 2012). The murmurs are normally caused by the rough or turbulent flow of blood near the heart or through the arteries. Systolic murmurs, in particular, occur as a result of heart muscle constriction and may reduce blood flow to the heart in the process (Lilly, 2012).Therefore, a detailed examination of a patient with abnormal heart murmurs is crucial since its main causes are the same as the causes of the other heart disorder conditions. Some of the causes include rheumatic fever, valve calcifications, the presence of holes in the heart septum and abnormalities of congenital heart valves. In addition, heart murmurs can also be caused, but temporarily by activities that increase heart beat such as anemia, fever, exercises, hyperthyroidism and pregnancy (Lilly, 2012).
Rheumatic fever, which is a streptococcal infection, for instance, may affect the heart producing scarred or inflamed heart valves. The fever may either remain or persist, and permanent damage may result in a disorder called rheumatic heart disease (Lilly, 2012). Congenital heart valve defects, on the other hand, is a birth disorder of abnormal heart development and can disrupt blood flow, making the blood flow slow, the wrong direction, rough or block completely. As a result of the above defects and the development of an opportunistic cardiovascular disease, unpredicted heart attacks and death may occur (Lilly, 2012).
