He has difficulty maintaining diet restrictions and managing his polypharmacy. He has 4+ pitting edema, moist crackles throughout lung fields, and labored breathing. There is no family other than his wife, who verbalizes sadness over his declining health and over her inability to get out of the house. She is overwhelmed with the stack of medical bills, as Mr. P always took care of the financial issues. Mr. P is despondent and asks why God has not taken him. Question Considering Mr. P’s condition and circumstance, write an essay of 500-750 words, in which you describe your approach to care, recommend a treatment plan, and provide both the patient and family with education. Congestive Heart Failure
Heart failure is a general term used to describe several types of cardiac diseases which lead to poor perfusion of tissues. Congestive heart failure is a progressive and debilitating disease that is accompanied by congestion of bdy tissues (Nair & Peate, 2009). The American Heart Association defines congestive heart failure, as a condition in which the heart cannot pump enough blood to the body's other organs. The “failing” heart is no longer working as efficient as it should.
Management of congestive heart failure involves both pharmacological and non-pharmacological management. Maintenance and improvement of the quality of life is the priority in the plan of care. Management includes cardiac rehabilitation. Cardiac rehabilitation is a medically supervised program to ensure cardiac patients the best possible physical, mental and social functioning so that they may, by their own efforts, resume and maintain as normal a place as possible in the community (American Heart Association, World Health Organization, 2005).
Lydia Hall clearly outlines the plan of care for Mr. P’s condition and circumstance. Patients with congestive heart failure have experience fatigue, difficulty of breathing and other physical symptoms that decreases both their physical and emotional endurance. The current trend in cardiac rehabilitation consists of clients and partners attending a group outpatient program conducted by a multidisciplinary team (Stewart, Inglis, & Hawkes, 2006). The patient may have physical, social, emotional and/or education needs depending on the severity and stage of his condition, knowledge of the disease, and current social support systems. In this case, given the fact that Mr P is frequently hospitalized to treat CHF symptoms, it seems that he and his family does not have enough knowledge about his condition and does not grasp the severity of the disease. His social support system is also limited to his wife who is overwhelmed by the problems brought about by her husband’s condition.
Utilizing Lydia hall’s Care, Cure, and Core Model, the nurse can devise a plan of care tailored to the needs of Mr. P and his wife. This theory, coupled with an accurate nursing assessment, the nurse can devise a plan of care for all the actual problems identified (Nair & Peate, 2009). The first thing the nurse has to do is establish a good and trusted working relationhip with the patient. Open communication and trust is necessary to facilitate care, provide education, and arrange discharge planning (Touhy & Birnbach, 2001). The Care Model means that the nurse is hands on with regards to patient care. This further strenghtens the trusting relationship the nurse and the patient has.
