Nursing situation
My nursing situation involves a new 54 year old uncontrolled diabetic male patient with PMH of hypertension, Hyperlipidemia, Chronic kidney disease stage 3 and BPH. During his second visit to review labs, A1C was 10.7, his med list reviewed and was on app.roprgpriate medications to manage his disease. Through investigation and counseling, patient reveals he was unable to afford his meds because he was uninsured. Patient was of Hatjan descent and was using herbal remedies to manage his disease. I completed the forms "for prescription assistance program and was able to receive his medications free of charge. I believe it’s important as providers to discuss affordability of medications with patients when implementing treatment plans.PICOT
(P) In Haitian American men between the ages of 50-65 with diabetes
(I) Does the use of medication questionnaire assist primary care providers to determine the
need for prescription assistance programs/medication compliance
(C) compared to patients who are not administered the medication questionnaire during office visit
(O) better management of A1C (T) within 12 weeks period
