This assignment requires you to provide an overview of the implementation plan for your project and how it fits into the healthcare system.  Start with your PICOt question and think through key players, specific tasks, quantifiable outcomes, etc. and how they all interact.
There are several examples of system maps in the Haines text. I suggest using the A-B-C-D Model.  It depicts inputs, throughputs, outputs, feedback loops, and sustainability mechanisms within the system. It should identify stakeholders and assessment points.  Your challenge will be to keep it simple and uncluttered. You will find two samples below.

For the Hispanic women between the ages 55 and 65 that are suffering from diabetes mellitus two, it is crucial that they stick to their medication. Failure to adhere to their medication will lead risks of increasing their blood sugars and developing further complications (Xiang, Kawakubo, Trigo, Kjos& Buchanan, 2010). The main huddle is to prevent the women from developing major complication by introducing the Glucose Buddy use, to increase the adherence to the medication and increase patient satisfaction(Glucosebuddy.com, 2015). Thismarks the output stage of the project otherwise referred to as stage A with reference to HainesA-B-C-D model(Haines, 2015).
Considering the aspects of fitting the app use into the health care systems, the number of patients that are going to use the app is going to be compared with a control group that will not use the app. This will give a quantifiable outcome measure to establish the success rates and the levels of satisfaction among the two groups. This will lead the project into phase B which relates to the feedback of the intervention(Haines, 2015). Using the results of the two interventions, the physicians, nurses, mobile app developers, insurance companies and local government health care official will be in a position to polish up the app intervention and see how best the patient can be assisted with their health condition(Xiang, Kawakubo, Trigo, Kjos& Buchanan, 2010).
